Are you a fitness professional, coach or trainer and need to cover your (probably-super-toned) butt?
This comprehensive fitness liability waiver release template is your first line of defense against an injury lawsuit. Note that a waiver typically CANNOT protect you from your (or an employee’s) gross negligence (lawyer nerd speak for “Crazy dumb stuff you let happen when you knew better”), meaning you should have proper systems, procedures, and training in place to make your business and premises are a safe environment for all your customers and team members. However, this waiver protects you as much as a piece of paper can.
You can have participants sign print versions in person or sign this waiver electronically (both are valid). Make your life easy with form signature software like Jotform , just make sure it or any other software (including a gym management app) or CRM authenticates the user and keeps a record of signatures. If you are working 1:1 with Clients, then add this waiver to the Coaching Agreement below so Clients have one thing to sign/e-sign!
